Under the direction of the Director of Clinical Nutrition and the Diabetes Program Coordinator and within established hospital and departmental policies, is responsible for promoting diabetes self management skills for inpatients, outpatients and during group classes.
Plans and administers diabetes education and management for people with diabetes in accordance with licensure and scope of practice. The diabetes educator works in conjunction with the care team and/or the primary care providers to promote self-management skills and optimize blood glucose control through lifestyle changes and medication recommendations.
This is a per diem position.
Education: BS in Dietetics
For RD: Must hold active registration with the American Dietetic Association Commission on Registration. Must be licensed by the State of New Hampshire.
Experience: Experience working in and both inpatient and outpatient setting preferred. Preferred Certification by the National Certification Board of Diabetes Educators.
1. Assess education needs of patients with diabetes.
2. Analyzes data to determine an assessment of the patient's clinical and functional problems, as well as the potential for improvement.
3. Develops realistic long and short-term functional, measurable, and objective for diabetes self-management.
4. Documentation of assessment and goals in the EMR and/or report to referring PCP.
5. Conducts follow-up visits and re-assessments at appropriate intervals
6. Runs group classes for Diabetes Self-Management.
7. Uses hospital scheduling system
8. Performs other tasks as assigned
9. Attends and completes annual hospital training
10. Participates in department QI and customer service initiatives
11. Attends work as scheduled and reports absences one hour before due
12. Observes the hospital dress code
13. Performs other tasks as assigned
14. For RDs: Adhere to Academy of Nutrition and Dietetics Code of Ethics

The check will bounce and the victim is left responsible.
